This is the confounding medical case of the girl with two immune systems.  In 2008, 9 year old Demi-Lee Brennan received a liver transplant. 9 months later her blood type completely changed. She was becoming her donor.  Her body was replacing her entire blood cell system with her donor’s.every This is the only known case of complete hematopoietic chimerism and it saved her life. It’s also the only known case of operational tolerance: the ability to receive a transplant without immunosuppressive drugs - a method transplant doctors have been trying to achieve for years. The human body is incredible. CLARIFICATION: A lot of people were asking how is it even safe/possible for an O RhD-negative recipient to receive an organ transplant rom an O RhD-negative donor. Based on the NEJM case study and my own understanding, the RhD antigen issue is primarily critical for blood transfusions and pregnancy, but less so for solid organ transplants. RhD antigens are on red blood cells and while there are some red blood cells in the vasculature of the organ, it’s in a small enough quantity where the benefits outweigh the risk. In certain critical scenarios like Demi-Lee’s, a RhD mismatch is acceptable given the clinical urgency. But, the RhD change that happened later (when her blood became RhD-positive) was a result of the chimerism, not a transplant complication.
