I think it was definitely FEV but he's obviously not gonna be a centaur, unless they ditch the canon completely.

There's actually a quest in 2004s Brotherhood of Steel game called "Find Missing Pieces" given by Harold. In the quest you need to find Harolds toe, eye and ARM. Harold is also one of the only examples we have of a subject being mutated by FEV but not submerged into it (Harold was just in the base inhaling the fumes, possibly had a little splash onto him). Thinks its FEV but hes becoming his own mutation like Harold, with similar experiences.

The show’s canon and they’ve been pretty careful about sticking to Fallout lore so far, so I don’t really see them changing this. Centaurs come from FEV vats with multiple organisms fused together, and Thaddeus is still just one person mutating over time.

I think we are looking too much into it. The guy f chickens and robots. I think he mixed all of those chems on that shelf and made something new. We don't see him doing anything pointing towards his medical knowledge or being intelligent. Thaddeus is growing a mouth on his shoulder so it is something new imo

don't forget the show is set years after the latest fallout game, somebody could've created an FEV potion that turns people into centaurs.

There’s a theory that since the dog licked his injured foot and that was enough DNA to becoming a centaur I don’t like it but it makes some sense

I'm guessing vault 32 and 33 were/are planned to be subjected to the FEV and be turned into super mutants but gotta wait til next week to be certain !
