This is a severe asthma attack — the airways tighten and swell so much that the usual blue "reliever" puffer (a quick-acting medication that normally opens the airways within minutes) can't keep up. The thing the cartoon is built around is real and widely taught as a patient red flag: if you're too breathless to finish a full sentence, if you're using your neck and shoulder muscles just to breathe, or if your blue reliever isn't working or you need it again and again, that's a sign the attack is severe and it's time to get emergency help — not to keep waiting it out at home. A note for accuracy: a loud wheeze can actually quiet down as an attack gets worse (less air moving), so "the wheezing got quieter" is not always reassurance — breathlessness, speaking in short broken phrases, and a reliever that stops helping matter more than how loud the wheeze is.
