at my department where I have been for 34 years you came in and ask for a complaint form they handed to you and you do what you want with it they don't ask you all these questions they don't need to know it only the one person who is investigating an internal affairs needs to know anything else they're trying to block any complaints before they get to internal affairs which is unethical at a minimum and violates the police officers code of conduct and probably is illegal and if it's not it should be because how can you make a complaint and expect it to get handled correctly if people are going to try to intercept it and stop it from going forward
