Yes, we actually read the whole chart. 2. Translate medical language into codes. We turn “acute on chronic systolic heart failure” into precise, compliant coding. It’s translation, not guessing. 3. Double-check documentation like detectives. Is it documented? Is it specific? Is it supported? We don’t assume, we verify. 4. Cross-check guidelines. ICD-10-CM rules. CPT/PCS guidelines. Edits. Compliance. Because “close enough” doesn’t survive audits. 5. Fix problems before they become denials. Missing laterality Unspecified diagnosis? Unlinked conditions? We catch it before the payer does (hopefully 😅). 6. Send queries to providers. Professionally saying: “Hi doctor… I need clarification.” Translation: Please save this claim. 7.
