I tell my guys when you take a bolt loose you own it. That’s YOUR bolt.It is no one else’s responsibility to tighten a bolt that you loosened yourself. I cannot be convinced otherwise.

It's going to happen, but you can reduce the potential with several changes to your routine. 1st & foremost, DO NOT interrupt a worker who is in the process of installing wheels & lug nuts FOR ANY REASON. Walk away, return, remember is one of the easiest ways to forget. Create a policy not allowing anyone to walk away from the vehicle once the wheel installation process has begun. Also do not allow conversation to take place with someone installing wheels. Allow their mind to have the full potential of focus during that step. You really can fully eliminate this issue with these steps even with semi-competent workers. If a wheel installer absolutely has to stop mid installation due to urgent bathroom break or injury or whatever. Immediately discharge that person from that particular installation & assign a fresh person to come start it over making them the sole responsible person for the install. Design that process to suit your desire but I recommend it be loosening & retightening all wheels. I've seen so many of these situations be caused from a service writer sticking their head out the shop door interrupting the worker for information so they can finish completing a ticket to cash out the customer while the worker gets distracted in the summary of conversation causing their mind to track differently & then turning around & looking at what they were doing, and for whatever reason in that exchange of interruption, overlooking part of the assembly process. People take for granted how simple putting a wheel on is, when something that routine gets interrupted for any reason, it's a recipe for disaster. That includes someone stopping to check their phone real quick while do that very task. Make it a rule that the phone, earbuds, watches, whatever is in the toolbox during the wheel installation process. Distractions are the NUMBER 1 cause of this issue. Period.

We have a policy that whoever let's the rack or the jack down, has to bump the wheel with their palm or fist. Every time.

worked in a UHaul shop and they checked the lug nuts before closing out the ticket and if they weren't right, they rolled it back into your bay and made you stop what you were doing and redo them.
