It prevents clots from getting bigger and stops new ones from forming. : Usually a weight-based bolus ➡️ then continuous IV infusion (units/kg/hr). It’s a HIGH-alert med, so double checks always. Monitoring: • aPTT every 6 hours : What to watch out for 👀: • Bleeding gums • Bruising • Hematuria • Black/tarry stools • Hypotension + tachycardia (internal bleed?) • Platelets 👀 — think HIT if they drop 50% or more : Antidote?
