IVF doesn’t replace preconception care. If anything, it makes it more important. When I support a couple preparing for IVF, I don’t just focus on the woman. I’m looking at the health of the egg, the health of the sperm and the environment we’re ultimately asking an embryo to implant into. IVF is an incredible technology, but it still relies on biology. The eggs and sperm still provide the biological material that creates the embryo. That’s why I want to understand what is happening with both partners before an IVF cycle begins. For my couples, that can mean reviewing pathology, nutrient status, hormones, metabolic health, diet and lifestyle factors, as well as taking a much closer look at male fertility. And one semen analysis doesn’t always tell us the whole story. Semen parameters can fluctuate, which is why repeat testing may sometimes be clinically useful when we’re trying to understand what is actually happening. Ideally, I love having around three months before IVF to work with a couple. Not because we can guarantee an outcome, we absolutely cannot, but because it gives us an opportunity to identify modifiable factors and optimise health before treatment begins.
