Buphthalmos is progressive. Causes of Buphthalmos: 1. Primary Glaucoma (most common) • Inherited narrow/closed iridocorneal angle (goniodysgenesis) • Breeds: Cocker Spaniels, Basset Hounds, Samoyeds, Chow Chows, Shar-Peis and others 2. Secondary Glaucoma • Chronic uveitis – inflammatory debris blocks aqueous outflow • Lens-induced – luxated lens or hypermature cataract • Intraocular neoplasia – e.g., melanoma, ciliary body adenoma • Intraocular hemorrhage 3. Congenital Glaucoma • Present at birth or develops in the first months Clinical Consequences: • Corneal striae (Haab’s striae) — breaks in Descemet’s membrane • Lagophthalmos, exposure keratitis, globe rupture • Retinal and optic nerve degeneration (irreversible blindness) • Chronic pain Prognosis: Poor for vision.
