Have you tried changing each item of tack one by one to see if something’s bothering him? Different style of bridle, different bit (or even try bitless), bareback etc. just to eliminate him being unhappy about something to do with his tack? X

Be wary that Bute trials do not eliminate pain behaviour as the pain can present differently and for a number of different reasons, in the same way that not all painkillers help every type of pain in humans

Equioxx is prescribed mainly as pain relief for arthritis, so is the vet thinking he has arthritis in his neck which is causing the head shaking??

Have you looked into Trigeminal head-shaking? my horse has it and it only happens in spring and summer for her but it’s extremely uncomfortable for her.

Have you had the dentist or vet out? Could be a bad tooth affecting the sinuses (sinusitis) I know our bran head shook bad when he had sinusitis..? Just a thought

Sounds daft but does he have ear plaques? I have seen one before that head shaking started when warmed up. It was the ear plaques causing a problem when he got warm

my mare was a head shaker. Hers is caused by seasonal allergies but als the surroundings (rape seed etc) she has to be ridden in a nose net, her bridle avoids all of her nerves on her face, and she’s on 20 anti histamines from now until October. It took about 3 months to figure it out and involved a CT scan (she ended up with sinusitis as a result of her allergies) and I had to move yards.

You probably have tried this but I’m still gonna say, my horse used to headshake like maaad! It was because his bridal would pinch his ears and his bridle path needs to stay super short otherwise he can shake a bit, just an idea. You’re doing so well with him! X
