I had to get a root canal back in 2020 and my insurance covered root canals but NOT FOR THE TOOTH I NEEDED IT DONE ON. Ridiculous.

My office refused to do a predetermination for my service. The office called but would not get prices bc they didn't do a pre determination. They never told me they didn't do the pre determination until a month before my surgery and they complained they couldn't get prices or a good estimate bc they did not do this. So I called and got some in fo but I couldn't have not gotten info or prices. They had two months to file that to get info with more accurate prices for the estimate

Pre auth and pre deter are also needed in other medical/surgical services. They always have this annoying disclamer that pre auth is not a guarantee of payment. Pre determination will give you the guarantee. Doing these two things are frustrating.

Would like to also bring up it also depends on the contract/portal submission as well. Meaning some offices are directly contracted and use the direct portal where some offices do not use the direct portal’s which they use third party submissions which can delay getting to insurance. The other thing about pre-auth submissions which is great is peer to peer review request asked by the office requesting to have review further if they do not agree. Most of the time when we as insurance get the call from providers/ third party’s on pre auths some of the information is missing x-ray, narrative, other supporting documents. Sometimes the denials are guidelines or tooth restrictions or missing tooth cause. Sometimes info can be found in provider manuals or the website of the insurance.

Not a guarantee of payment plays a vital role for all insurances, since an auth or pre d was approved (safe to say they reviewed and investigate the records) then why are they still put this disclaimer whenever they provide info to providers.
