Here’s How Clinicians Test It 👀 As a board-certified Orthopedic Clinical Specialist PT, one neurodynamic test I frequently use when evaluating arm pain or suspected cervical radiculopathy is the Upper Limb Tension Test 1 (ULTT1) which biases the median nerve. This test progressively loads the nerve to see if symptoms are reproduced. ◻️ Why it helps ULTT1 can help identify neural mechanosensitivity or possible cervical nerve root involvement when patients report: • Arm pain • Tingling or numbness • Referred symptoms down the arm It’s commonly used as part of the Wainner Cervical Radiculopathy Cluster to improve diagnostic confidence. ◻️ How the test is performed 1️⃣ Scapular Depression The therapist gently depresses the shoulder to tension the brachial plexus. 2️⃣ Shoulder Abduction (about 90 to 110°) The arm is moved away from the body while maintaining scapular depression. 3️⃣ Shoulder External Rotation 4️⃣Forearm Supination 5️⃣ Wrist and Finger Extension 6️⃣ Elbow Extension The elbow is slowly extended until symptoms appear or resistance is felt. Structural Differentiation The cervical spine is side bent away from the tested arm. If symptoms increase, neural involvement becomes more likely. 💡 Clinical pearl The test should be performed slowly and sequentially. We are not forcing the arm into position.
