Follow the RN supervisor instructions and then clarify with MD. Then document. You should always clarify with MD regarding medical orders before making your own decisions, especially when a RN delegates or gives instructions. Otherwise, it could be followed-up as failure to follow delegation or worse. Thats how you best CYA. Trust, its in your best interest due to the legal structure of our scopes of practice.

Yes! I’ve been an LPN for 30 years. Goal is to keep comfort on an even level. If you don’t give scheduled comfort meds they won’t remain comfortable.

No she was supposed to call her hospice nurse bc hospice is who is directing her care. Going forward should that happen just call hospice and make them aware of the situation and they will send her hospice nurse out to evaluate and change the order if they deem it necessary.

Hospice nurse here. Every person I have ever cared for who has passed away through the dying process chooses their time. I cannot emphasize that enough. The meds will not speed it up as long as they're given in the parameters set by hospice. The end result is inevitable, how difficult it is—that's controllable and up to us. They deserve the dignity to work through whatever they need in this life to allow them to let go and should NEVER have to struggle to do that. Thank you for your compassion to that, it's hard to change a person's mind when they have that stigma looming.

Uhhh, it sounds like that "supervisor" is practicing medicine without an MD license. Usually hospice physicians, contracted by the facility, change the orders... Not nursing supervisors.

Staff nurses aren’t supposed to deescalate hospice care because hospice is usually managed by an outside agency. They’re also not supposed to change orders without speaking with the provider on call and usually something like this would wait until later.

The idea is to get ahead of symptoms instead of chasing them. It doesn’t “seem like it’s needed” because they have been getting those meds, and they are COMFORTABLE.

As a RN Case Manager for hospice…always give the meds as ordered. If your RN manager felt like the pt needed a medication change she was supposed to reach out to hospice so a nurse could be sent to reassess the patient. You did a good job!!💜

As a hospice nurse, I’m having a huge problem with a local assisted living not giving scheduled pain meds at the end of life. Straight up refusing to and it’s devastating for the patients who are suffering and their families who are watching them suffer. It’s terrible
