@im.nurse.liz: Replying to @Ashley J. We can’t fix this healthcare system by working off the clock and providing sub par care, or burning ourselves out. We have a very messed up healthcare system: and it will steal everything you are willing to give. Don’t be me. Keep those boundaries! #nursepractitioner #primarycare #familymedicine #healthcareworkers
Overcompensating on your end is enabling behavior that allows a dysfunctional system/boss/job/etc. to continue said behavior

When patients start the appt complaining about the wait list, I thank them for their patience and ask how much of our remaining 10 mins together they’d like to spend talking about my wait list.

You get the next available. It’s all there is. I can’t fix that

Our schedulers suck butt, too so inconsistent I’ll want someone to have an asap appt and it will be booked 3 weeks out and then the adult with 2 days of sniffles gets a work in.

They could get a half ass answer in the portal or a REALLY productive visit with education and expertise for a better overall experience.

I work in GI. I have 20 a day on my schedule and am still 8 weeks out. We have such a shortage of GI providers to support our population. There’s no room to try to get someone a next day appointment. I’m not working 10 hour days just to support the load of a broken system.

I’m the only nurse working for a provider with a 2300+ patient panel and we’re booking out until July. We add on 2 patients per day every day minimum and we simply can’t do it anymore. Every day I’m dodging a handful of calls from patients wanting to only see us.

Boundaries are so important!! It took me 17 as a primary care NP to learn this. Thank you for this honest answer 🙌🏼, it will help many to not burn out 🔥. Protect your peace fellow PCP’s 😌✨🕊️

Ah the system relies on us wanting to overcompensate though! 😅

My PCP is very responsive and also, I really don’t like it when I see a portal message come in on a Sunday afternoon. I want her to be my PCP for as long as possible, not burn herself out.

I usually say something along the lines of “there are a lot more of you (patients) than there are of me, so I thank you for your patience and understanding as I try to help all of you equally and safely.”

The other thing is that you need to have reserve to deal with true emergencies. If my extra time is taken up, I want it to be for a reason that reduces burnout (being there for someone in a hard time).

How exactly do people expect the providers, who are literally just other humans and not robots, to fix this issue? Growth is NOT infinite with finite resources.

I previously would work through lunch or add patients on during my admin time and it got to the point where I was doing notes and other admin stuff for hours every evening. I was miserable and just couldn't keep doing it. It was either they wait for next available or I'm not available ever because I was going to leave if something didn't change.

Had a patient who wanted me to see her as an add on because I was seeing her husband. I was already behind and had a packed schedule. She had an appt already scheduled for the next day. I told her no. She proceeded to make comments about how she wished I would just seeing her throughout her husband’s visit.

Whenever people leave Google reviews mentioning the long wait (I’m in ENT), our admin responds by encouraging them to call their representatives to complain because there is literally nothing we can do about this. I will not over extend myself because there aren’t enough providers in my state because the reimbursement is so bad. People need to hnderstand that this needs to be addressed at a legislative level, not a practice level.

Then don’t have an insane amount of patients that causes the long waits. You as the provider sign a contract with a healthcare clinic. You set the standard for what you feel is a safe patient load. Yes office managers have a part in scheduling but you as the provider have the ability to set patient case loads.

You can fix it. Not your office manager. Or they will fix it with 5 minute appointment slots. You are the money maker. Not the OM or nurse or MA or even the MBAs. You. So you collectively take that shit to the C-suite and make the changes needed to better take care of your patient load.

Well one problem that could be fixed by the patients/clients themselves is to not no call/no show and/or cancelling giving the place enough notice to potentially fill that spot. I work at a vet and we’ve been having a lot of no call/no shows even appointments that were confirmed. When you don’t cancel at all or don’t give enough notice, that spot you took up is now wasted because how am I supposed to fill that spot when you cancelled 10 minutes before your appointment. And I think we all understand emergencies and unexpected events happen that may have caused you to cancel but this is more so for the people that constantly do it. Please stop
