Let’s talk about systolic blood pressure (SBP) goals in patients with spontaneous intracerebral hemorrhage (sICH). (May as well start head to toe with a neurology topic 🧠) There are 2 main types of strokes: ischemic and hemorrhagic. Hemorrhagic strokes occur less often but are associated with worse morbidity & mortality. BP control is an essential aspect for management of hemorrhagic stroke. As a reminder, sICH is a hemorrhagic stroke due to etiologies like hypertension (HTN), cerebral amyloid angiopathy (CAA), medications/drugs (cocaine, amphetamines, blood thinners), & arteriovenous malformation (AVM). Why is SBP control so important in sICH? Even if sICH is not HTN etiology, patients may experience high BP due to mass effect & increased intracranial pressure. High BP increases risk of hematoma expansion (HE) especially within the first few hours after ICH & is associated with worse outcomes. Specific guidelines for sICH were posted by AHA/ASA in 2022. Two main studies (INTERACT2, ATACH2) provided data to recommend specific BP goals in patients with sICH. Guideline Recommendations: ▪️Suggest SBP goal of 140 mmHg (range 130-150) for patients who present with SBP 150-220 to improve functional outcomes ▪️SBP < 130 mmHg may be harmful due to decreased cerebral perfusion pressure (CPP) & adverse renal outcomes including acute kidney injury (AKI) ▪️Data suggests worse outcomes in patients with SBP variability within 24 hours of ICH, therefore guidelines suggest avoiding SBP variability during 24 hours emphasizing smooth blood pressure reduction ▪️Guidelines suggest reaching SBP goal within 1-2 hr of ICH to reduce HE & improve outcomes (Keep in mind sICH is an exception to the BP management in hypertensive crisis, so recommendation in HTN crisis for mean arterial pressure (MAP) decrease by no more than 25% in 1 hour does not apply to sICH unless per provider discretion) ▪️ Available data did not include patients undergoing neurosurgical evaluation so the safety & efficacy of intensive blood pressure lowering in these patients is not well established Stay tuned for more content! #stroke #ich #intracerebralhemorrhage #neuroscience #neuro #neurocriticalcare #neuromedicine #healthcare #medical #bloodpressure #medicine #criticalcare #pharmacy #pharmacist #brain #brainbleeds
