Also good to keep in mind that digoxin is not fast acting and will take some time to work! They might sit there tachy for a little bit. Dilt IVP +/- an infusion is also quite effective for fast rate control as an alternative to metop

Amiodarone has very good rate and rhythm control. Usually A-Fib with RVR is dangerous because they have an increase risk of cardioembolic stroke. So usually the first thing in management is Anticoagulation

diltiazem for rate and amiodarone for rhythm, but you're right.....Amiodarone is a "dirty drug", not good at all for long term control.

I am currently studying for Step One but i came across a Tik Tok stating the 48 hour rule for AFib. If >48 hours begin anticoagulants and rate control (if hemodynamically stable). You need to check for thrombus in atrium prior to synchronized cardioversion. If 48 hours and follow treatment as mentioned above. Like I mentioned, saw this on another Tik Tok a few months ago, I don’t know if this is proper protocol. 😅
