People from the U.S. say “American” because it comes from the country’s full name—the United States of America. There isn’t a normal alternative, and nobody is about to say “United Statesman” or “United Statesian,” so “American” just stuck. The confusion is really just geography vs. everyday language. Yeah, North and South America are continents, but no country is actually called just “America.” The U.S. is the only one with it in the name, so in regular conversation, “America” usually just means the United States.
