Here in Canada, I needed detailed information about my disability filled out by my family doctor so I could apply for disability student funding. He charged me 200 dollars and said he would have the paperwork ready in 2 weeks. 5 months go by of fighting for the paperwork back, I got it just to find he removed 4 pages that required actual understanding of my condition and a few sentences of description. He tried to charge me another $200 when I asked him to fill out those pages. He claimed I never brought those pages, but I took pictures inside his office of the pages he removed before I handed them the paper work. He got one of his nurses to fill the rest out. Someone I never met. The form was filled in French even though I don't speak the language and they tried to charge me again when I picked up the final copy. They insisted they had to keep the original for legal reasons. The copy job was terrible, barely readable. It took them so long to fill it out that I couldn't apply for the disabilty funding and had to pay a lot of money out of pocket to my college as a result.

I had to get a doctors note about my anxiety he sat me down and asked if I was just being overly emotinal or if i just wanted attention!! IMMEDIATLY LEFT
