Nah I don’t like when providers do this because this is how notes become inaccurate. I’m not saying you but I be seeing notes charting they did exams that they did not. Things I did not say, ect. Unless I’m inpatient (cause I don’t really care, I ain’t reading all them notes from a 8 day stay in the hospital; discharge summary will suffice). 😆 But if I’m in clinic or ED please try to complete my chart within the next few hours. 💕🤗
