1️⃣ Osteosarcoma Classic findings are sunburst periosteal reaction and Codman triangle. Aggressive bone tumour. Mostly seen between ages 10–25. 2️⃣ Ewing sarcoma Onion-skin appearance on X-ray (multiple thin, parallel layers on the shaft). Aggressive bone tumour. Mostly children and young adults. 3️⃣ Multiple myeloma Multiple punched-out lytic lesions, mostly affecting the skull, spine, ribs, pelvis and long bones. 4️⃣ Chronic osteomyelitis Mixed bone destruction and sclerosis. Look for sequestrum (white dead bone inside dark infection) and invoculum (white new bone shell around it). 5️⃣ Brodie abscess A well-defined lytic lesion (dark) within the bone, usually surrounded by a rim of sclerosis (white). Often mistaken for a bone tumour. 6️⃣ Avascular necrosis of the femoral head Loss of blood supply causes sclerosis, flattening and collapse of the femoral head. 7️⃣ Pathological fracture A fracture occurring through abnormal bone. Clues include an underlying lytic lesion, cortical destruction, or abnormal bone architecture at the fracture site. 8️⃣ Femoral neck fracture Commonly after a fall in elderly with osteoporosis. Missing it can lead to loss of mobility, avascular necrosis, and significant morbidity. 9️⃣ Giant cell tumour Expansile lytic lesion located near the end of a long bone, most commonly around the knee. Although benign, it can be locally aggressive and weaken the bone. 🔟 Bone metastases Lytic or sclerotic lesions caused by cancer spreading to bone. Common primary cancers include breast, prostate, lung, kidney, and thyroid cancer. 👉 Comment XRAY and I’ll send you the links I’ve used to practice.
