I had one patient last week on LR at 125 and lasix , when I asked the doctor if he wanted to discontinue the fluids since she’s on the diuretic he said no because she’s NPO. I wanted to say “so which is it? Do you want her dry or hydrated but you know we have to keep working with them so I try not to antagonize them

It is possible for the patient to be intravascularly dry but be fluid overloaded as a whole as fluid moves out of the vascular space and into cells or extracellular space. AKA “third spacing.” IVF moves out of the blood stream somewhere in the realm of 30-120 minutes. Also the IVF helps to protect from AKI. Electrolyte replacement in the IVF can also be a factor
