Adult Oblex its a hyper-intelligent, Lawful evil ooze that can perfectly impersonate anyone it has stolen memories from. This is automatic on a failed DC 15 wisdom save, you know, the stat you dumped for higher strength and dexterity. Its basically John Carpenters 'Thing' movie in D&D. Absolute paranoia if played correctly.

Hear me out, not an actual monster but a big scar. A NPC who sells the party over powered gear, new magic spells or something meta like a level up but it’s for surprisingly cheap the NPC says you’ll pay in other ways after the party buys it then they will be scared for the next few sessions how they’re gonna pay, the trick is it never comes up again

Not an unheard of monster but an evil way to run one: A Lich but the phylactery is a friendly NPC that the party took under their wing and have grown close to so they only way to slay the Lich is to kill their friend.

The Gospodin. Basically, I took "The Gentlemen" from Buffy the Vampire Slayer; and made them an unstoppable "omen" type monster with support monsters, that you have to solve a puzzle to beat them... And if you don't, entire villages go missing overnight... Worse yet, they are an omen to Ravenloft. If you survive but don't beat them, you end up in the Isles of Dread.
