This is a really typical example of lateral frontalis overactivity. The centre of the forehead has been softened by Botox, but the outer fibres are still active, so they compensate and bunch up just above the brow tail. The frontalis is the only brow elevator, so if the sides are left stronger this is exactly what you see. It’s not the corrugator supercilii muscle as that pulls the brow down and in.
A lot of the confusion in these comments just comes down to not fully understanding how these muscles balance each other. This is a simple tweak at review with a small, precise lateral adjustment
