The co-pay is too much for a lot of people, $200+ per day, so saying Medicare only pays for 20 days is accurate for them, and many people leave because of that

well, you have to meet Medicare skilled criteria for one. That is dependent on how many days you remain skilled for and clinical indications for a skilled stay is very minimal so unless you have IVs or some critical occurrence is happening, likely you won’t be sealed for skilled nursing needs and probably just mostly therapeutic needs and if your menace assist or even stand standby or contact artist assist, you probably aren’t meaning skilled criteria either. Medicare does not consider or look at or include their criteria in your own home environment, issues, or lack of support issues or dynamics of family or circumstance it’s only purely going to look at. Can you be managed from an outpatient perspective and is another week going to make a marginal difference in your abilities..

Always appeal. Former nurse case manager here. When I would let patients know that the doctor felt they were ready for discharge, I always let the patient know how to appeal. Always appeal!

I’ve worked in SNF in rehab for 25 years. Most people want to leave at 20 days because of the co pay. Facilities love keeping people for the whole time. Now, manage care Will want the patient out ASAP. Many patients appeal some win some lose. That’s beyond the facility unless the patient wants to pay privately.

I wish I knew this when my dad was admitted to rehab and then coverage “ended” about 2 weeks later. They even started the clock when he got to the rehab facility on a Friday night but his rehab did not start until Monday (plus he had no rehab on weekends). I wish I’d known this because then we had to transition to self pay for the remainder of his life. I think he gave up when they told him he no longer qualified for rehab.

At this level of care, Medicare Advantage plans are a problem compared to Traditional Medicare. You do have to be aware of expensive copays at day 21.
