How do you hold someone accountable and also answering who is at fault when asked? Sometimes saying facts is seen as putting the blame on someone.

For those asking how do we make them accountable? You should be mature enough to understand that you are accountable if it is your process. The leader shouldn't have to explain it to you. That is why he should not be asking anymore who to blame; anything worth punishing is HRs responsibility and keeping things on track is the leader's. And mind you, mistakes are part of the work. That is why its more important to make a safe space for employees to voice out their mistakes: committing it unintentionally with the best of efforts shouldn't be punished it should be worked on and learned from.

This is true naman, but easier said than done lalo na kapag extremely underperforming yung subordinate mo na kahit ulit ulitin mo kausapin wala nangyayari.
