@drjenashton: Big news in the world of Obesity Medicine and weight management today, and I want to make sure you actually understand what it means, not just that it happened. The FDA just approved Foundayo (orforglipron), Eli Lilly's once-daily GLP-1 pill for obesity, and it's worth paying attention to. Here's what you need to know: The clinical data: In the Phase 3 ATTAIN trials, patients lost an average of 12.4% of their body weight at the highest dose over 72 weeks. Real, meaningful weight loss. Less than Zepbound's ~21%, but this isn't a shot. It's a pill. The access piece: With commercial insurance, eligible patients could pay as little as $25/month. Self-pay starts at $149. Medicare Part D coverage could begin as early as July at $50/month. For a category where people have been paying $1,000+/month, that matters enormously. The practical difference: Unlike the Wegovy pill, which requires an empty stomach and a 30-minute wait, Foundayo can be taken any time, no food or water restrictions. Side effects: Same GI profile as the rest of this class. The thyroid cancer warning is there too, so if you have a history of medullary thyroid carcinoma, this isn't for you. Is it a replacement for the injectables? No. A legitimate option for people who want oral medication, or an oral maintenance strategy after hitting goal weight? Yes.
