SLR with perturbations - restoring terminal knee extension and quadriceps control with intention. 🔬⚡️ In rehab, the Straight Leg Raise (SLR) is far more than a “basic leg lift.” I use SLR variations with perturbations and tactile quad tapping to improve terminal knee extension, quadriceps recruitment, and neuromuscular control. The primary goal of the SLR is quad activation ESPECIALLY after knee injury or surgery when swelling, pain, and immobilization create quadriceps inhibition and disrupt the brain-to-muscle connection. Before the leg lifts: ▫️The quad is “set” first ▫️The knee is locked into extension ▫️The contraction is maintained throughout the movement This helps: ✔️ Re-engage the quadriceps ✔️ Improve knee stability ✔️ Maintain strength with minimal joint stress ✔️ Prevent extensor lag ✔️ Restore confidence in terminal knee extension I then add manual perturbations and tactile tapping to increase proprioceptive input and reinforce sustained quad activation under changing demands. Why perturbations? ▫️Challenge reflexive stability ▫️Increase neuromuscular demand ▫️Expose compensatory strategies ▫️Improve motor control and positional awareness Especially valuable after: ▫️ACL reconstruction ▫️Total knee replacement ▫️Patellar injuries ▫️Meniscus surgery ▫️General quad inhibition from pain/swelling The goal isn’t just lifting the leg, it’s teaching the quadriceps to maintain terminal knee extension confidently so it transfers into gait, stairs, deceleration, and higher-level movement. 💪
