@nclex.review1: #nclexprep #nursingstudent #nursing #nclextips #fyp This is a priority NCLEX question — think: What is happening? What can kill the patient first? Key Clues: • Restlessness → early sign of hypoxia or shock • BP 88/50 (LOW) → hypotension • HR 130 (HIGH) → tachycardia This screams SHOCK, most likely post-op hemorrhage ✅ Correct Answer: C. Assess for bleeding Why this is FIRST: On the NCLEX, you always follow: Assess before you act (unless airway is compromised) • The patient is showing signs of hypovolemic shock • The most likely cause post-surgery = bleeding • You must confirm the cause FIRST before jumping into interventions Why the others are wrong: • A. Call the provider Not first — you need data before reporting • B. Increase IV fluids Might be needed, but you must assess the cause first • D. Administer pain medication Pain meds can worsen hypotension → dangerous here NCLEX Memory Trick: “Restless + Low BP + High HR = THINK BLEEDING FIRST” Exam Tip: Post-op + sudden deterioration = Check the surgical site, drains, dressings immediately
All of you are quick to guess the answer but theyre all wrong. First things first, wash hands then confirm DOB

Chioma | With God (RN) :
Assessment has already been done to arrive at a BP of 88/50 and HR of 130. This is a case of hypotension.
The nurse's priority action is to increase IV fluids.

Feels like assessment was already done, that’s how we know the vitals. I would increase fluid. And monitor.

Syn Uwa :
Assess first, call provider, implement orders. Increasing IV fluids without an order is out of the scope of practice for an RN I Believe

Actually because it's post surgery you would assess for bleeding because that can cause for the drop in BP and tachycardia.

Not your average Karen :
You always have to assess first . You’re going go right into treating a pt before assessing? I’d rerun b/p in different location, assess for a source of bleeding (surgery), is my pt symptomatic?, notify doc of my findings . On my unit 88/50 is not a terrible b/p.

In reality B. But if by the book since this is NCLEX it must be C. Assess first, even though you know that the client is on state of shock because of bleeding or something.

KATAKANA BEAUTY SHOP :
A: Calling the provider is important, but you need to have assessment data (e.g., active bleeding) to report first.
- B: Increasing IV fluids is a treatment for shock, but you must confirm the cause (bleeding) and assess first—this is an intervention, not a first step.
- D: Pain medication is unsafe here (low BP makes it risky) and irrelevant: pain typically raises blood pressure, not lowers it; restlessness here is from shock, not pain.
