Uh the Khan made up the entire system and rules that Librarians need to follow Magnus helped by saying how powerful he is and how he's better than everyone else while Sanguinius offered useful suggestions

The Librarius Conclave was formed by Magnus, Jaghatai, and Sanguinius in a response to the moves of Mortarion and Russ to shut down psykers. Nikea happened and Jaghatai, Magnus, Russ all really ignored it and continued heavy Psyker use. The librarius was still utilized afterwards thanks to that effort

Sorta. Not exactly. Council of Nikea found the librarius to be dangerous. Thousand sons and wolves kinda ignored it. Leman burns prospero. Lots of dominos fall terra burns. Girlyman reinstates the Librarius shortly after/during the scouring
