Rural and underserved patients deserve inpatient physicians just as much as ones in cities and suburbs. Creating subpar access to care creates different tiers in medical treatment altogether, and patients deserve so much more than that. In a critical care setting, such as ED or ICU, there should be a physician physically present at all times. When other providers, such as RNs or APP‘s, don’t know what they don’t know, critical things can be missed that are time sensitive, and the patient pays the price. It’s unacceptable. Telemedicine is useful in certain stances, not in high stakes clinical settings such as remote supervision for EDs and ICUs.
