I know it’s annoying. But it’s done because people forget things and sometimes those “things” are big deals that will affect outcomes. Better to double or triple check than to end up injured because of an incomplete medical consultation.

We need updated information to make sure that we can always reach you, track your child’s milestones, flag early identifiers of families needing help with food/clothing/shelter but are embarrassed to ask … lots of reasons. It all goes into the chart so we can make sure we can provide great care and always reach out to you or so we can bill your insurance company accurately so they can’t deny you.

because your health changes all the time. We need to keep current and updated records for the best care of the patient. If you become allergic to a medication and the doctor prescribes that medication with out knowing your allergic who is at fault then? it takes 5 mins to fill out.

They are actually testing you. And if you worked in healthcare you’d know that so many patients give receptionists, nurses, and providers all different information and answers for the exact same questions. They will literally lie to the first two because they’re embarrassed or just don’t want to tell anyone else other than the provider. They may also give more information to one person and not the provider, so it makes care a bit better. Many patients don’t think certain things matter when it comes to their care or a diagnosis, but it can absolutely alter how your plan of care will be handled.
