The Jose Melena case. In October 11, 2012, 62-year-old Jose Melena was working at a Bumble Bee Foods plant in Santa Fe Springs, California, when he entered a massive industrial oven used to sterilize cans of tuna. Without realizing he was still inside the machine, his coworkers loaded it with around six tons of tuna, closed the heavy steel door, and started the heating cycle. The pressurized oven reached about 270°F during a process that lasted around two hours. When workers realized Jose was missing, they began searching for him around the factory and the parking lot. But when the oven was finally shut off, cooled down, and opened, it was already too late. His b**y was found inside the machine. The investigation pointed to serious safety failures, especially in the inspection of the equipment before it was turned on. Years later, Bumble Bee Foods agreed to pay a $6 million settlement related to the case.
