are you a carer working under the pressure? Or... you work in the office, having a good ventilation, time to drink your coffee, time to chat, etc.... So.....we should have 1 hour break to sit down and to write down all these detailes.... Sometimes we don't have time to use the facilities,..... "Reality" is different from what is written in books....

I write no concerns because there isn’t any concerns to write. Everything is well documented down to what they eat and drink and how they are feeling.

I write 'no concerns' after completing my notes.... And il continue to write 'no concerns' at the end of my notes.... But thanks for your input put.... 🤷♀️

I always write "no concerns" and that's because I have....well no concerns obviouslyif i did they would be documented. I also right "ate well" but i do write down what they have eaten.

Senior carer here, nothing wrong writing No Concerns when you have documented everything else, at the end of the sentence saying that fine by me, however as I said as long as you documented everything else xx
