Achilles tendinitis can be a very frustrating thing to handle. Since it happens in a weight-bearing joint, it can be difficult to continue your normal life while trying to recover from this. When a patient is genuinely in so much pain that they are limping - sometimes things like short-term pain management can be necessary. This most commonly includes things like massage, maybe dry needling if tolerated, heel lifts are seen in the literature as well. We want to start loading the tendon as soon as we can to put stress through it, but without flaring up the pain too much. Isometric holds are a great option at first because we can stress the tendon without doing repeated movement which can be irritating earlier on in the process. And whether it is with heavier and harder isometrics or actually doing reps of a calf raise, we want to continually and progressively load that tendon all the way to a heavy-slow loading protocol. ** This is where it can be important to work with a professional because it is difficult to know how much pain to push yourself in. Pain is a very real part of rehab, and it cannot be fully avoided and some research suggests that we should not fully avoid it if we actually want to help a patient. But knowing how to safely improperly work with the pain can take a lot of education and experience. * A heavy slow protocol basically means that multiple times a week you are going to do calf raises through a full range of motion, nice and slow, and we’re gonna make them nice and heavy. it is also important to understand that sometimes tendinitis happens because of a problem outside of that tendon and muscle itself. So we should not only focus on the calf, but also factors coming from above. You need to be fully evaluated essentially from the hips down to find other reasons why that tendon may have started to become dysfunctional. Often times these are exercises we can start right away, no matter how much pain you’re in. Cause you should be able to tolerate glute exercises without excessively stressing the heel The most common issues with recovery that I see are: 1. Having an unrealistic timeline. Just because you aren’t better in two days, doesn’t mean the treatment isn’t working. Keep going. 2. Not modifying your lifestyle. We need to talk about your job and home requirements and find ways to help you live your life without excessively stressing your heel. (This can be hard) 3. Failure to progress. This is usually the fault of the clinician. You have to be progressed and pushed to truly recover from this tendon issue. 4. Jumping ahead. You have one good day and you think you’re healed so you excessively stressed that heal and you cause a big setback. ##achillestendinitis##heelpain
