Pad all pressure points, chest, iliac crest 8. Protect G-tube, trach, VnS, pacemaker, Pain pump, SNS, J-tube or drains and implants if present 9. Position arms 90/90 if possible, else tucked 10. Secure abdomen free to reduce venous pressure 11. Drape field with Ioban, towels, wide exposure 12. Prep NM leads, confirm baseline MEP/SSEP 13. Position O-arm and stealth array reference frame 14. Verify registration surfaces are dry & exposed 15. Confirm navigation working pre-incision 16. Draw dotted midline, mark anatomic landmarks 17. Set Aquamantys 180W and Bovie 50 18. Begin midline incision T2 to sacrum 19. Cauterize subcutaneous tissue down to fascia 20. Subperiosteal dissection to spinous processes 21. Elevate paraspinals laterally to TPs 22. Pack Raytechs gently subfascially bilaterally with Cobb 23. Confirm adequate exposure to SI joints 24. Minimize exposure if bleeding heavily 25. Register reference frame to exposed spinous process which is through PSIS incision 4.3mm 26. Perform O-arm spin for navigation registration 27. Confirm accurate map with 3-point check 28. Navigate pelvic entry with eagles claw 29. Mark S2AI start between foramina 30. Burr entry, advance navigated awl to notch 31. Confirm tract with navigated probe 32. Insert guidewire, maintain under navigation 33. Tap with cannulated system over wire 34. Choose 8.5mm x 65–75mm SAI screw 35. Drive screw using navigated inserter 36. Confirm trajectory 37. Repeat steps for contralateral side 38. Save AP, inlet, outlet screenshots 39. Irrigate pelvis zone, repack Raytech 40. Return to L5, register screw trajectory 41. Burr cortical entry, start with curved awl 42. Cannulate pedicle with awl, minimal arc 43. Use wand to verify mid-body trajectory 44. Probe with ball tip probe 45. Probe with ball-tip, confirm five cortical walls 46. Measure depth wit
