This morbid conviction can become delusional and, over time, lead to dangerous behavior, including self-aggression (both mental and physical). Preoccupation with appearance in dysmorphophobia causes significant discomfort or problems in social, professional, academic, or other areas of life. The term "dysmorphophobia" was first coined by the Italian psychiatrist Enrico Agostino Morselli in 1886. He described the syndrome as "a subjective perception in the patient of a supposed deformity or physical defect that seems noticeable to others." Patients understand their problem differently, but usually the level of understanding is low or absent altogether, i.e., most patients are sure that the body part they dislike “possibly” or “obviously” looks abnormal, ugly, or unattractive.
