The root-cause - “a printing error erased that change” doesn’t really make sense. You might describe the cause as an information security issue - the integrity of the data was not preserved - an unauthorised change to the data took place, whether by human action or non-human error, and whether accidental or intentional e.g. malicious. I’m sure there would have been detailed investigation.

Equipment should always be checked before hand however it showed officials incompetence to notice there was something wrong with the fault. Just shows you the unprofessionalism of the organisers.
