This is very true. My insurance based PT discharged me when I met all functional daily living goals within 4 visits so I was discharged from PT. My MD was disappointed, but it was an insurance & healthcare system issue and not that PT’s fault. She referred me to a cash based PT clinic that focuses on ‘return to sport’ and bridging traditional rehab to get back to achieving athletic goals. It took 10 more visits and almost 3 more months of rehab but I’m finally back to running and kickboxing again. It was very expensive but I’m lucky that I was able to afford it. I did love my PT that reluctantly had to discharge me, she’d helped me for a previous injury, but I wish the healthcare system in our country was different.

I’m an outpatient OT and my favorite line when doc tells them they can be done with therapy (especially post op and we haven’t even started strengthening yet??) is “well I wouldn’t tell you to stop seeing doc for your post op appts so they shouldn’t be telling you when to stop seeing me” and that usually does the trick 🤣

You (or I) as the PT do not decide when PT ends, all we can decide is if we are willing to continue or stop at any time. How long a patient should be in PT is a group decision involving at least 2 people, the patient and the PT usually but if the physician has recommendations with clinical justification, it should be followed through with. As a patient progresses they go from PT definitely being needed to varying degrees of needing to continue, to PT no longer meeting medical necessity which is also a PT's opinion. Patient may have different and changing goals, a surgeon may want more strengthening before an upcoming procedure that the PT may not even be aware of and countless other scenarios. When to stop is a group decision based on many factors and often debatable.

Or when they say “I see the doctor next week so I’ll see if they think I should continue”.. like maybe ask the PT OR have autonomy over your OWN health and decide if it’s benefiting you or not!!!

I’m an OT at a SNF and I absolutely loathe when new admits come in and say “the doctor said I’ll only need to be here for two weeks.” But they end up being a hoyer lift, completely dependent for ADLs, and they live alone with 16 steps to get in the house…
