Replying to @Slicc One of the biggest mistakes I see on the TMC is students trying to memorize every pulmonary function test value instead of recognizing the pattern. Here’s the order I want you to remember: 🫁 Step 1: Look at the FEV₁/FVC ratio. A low ratio = obstructive disease because the patient has difficulty getting air out of the lungs. 🫁 Step 2: If the ratio is normal, check the TLC. A low TLC = restrictive disease because the lungs can’t fully expand. 🫁 Step 3: If the FEV₁/FVC ratio is low AND the TLC is low, think mixed obstructive and restrictive disease. ⚠️ TMC Gold Standard Tip: A low FVC alone does NOT confirm restriction. Severe obstruction with air trapping can also reduce the FVC. Always look at the entire PFT pattern before choosing your answer. Common diseases to remember: ✅ Obstructive: COPD, emphysema, chronic bronchitis, asthma, bronchiectasis, cystic fibrosis ✅ Restrictive: Pulmonary fibrosis, interstitial lung disease, obesity hypoventilation, kyphoscoliosis, neuromuscular disorders If your TMC is coming up, don’t just memorize numbers—learn the logic behind the interpretation.
