How would A know unless it’s not a secret ballot. I’d vote B and if A won I’d say I voted A and I would tell other B voters to do the same.

Picking A cause what if A wins, I'd rather be on the safe side cause B will still give me the pill whether or not I choose him but if I choose B and A wins then I'm screwed.

Assuming they have an otherwise equal platform aside from the cure, there is no reason to choose B. Reframed: if you want to live with 100% certainty, vote A. Why would anyone vote B if the rest of the platforms are the same?

B. I don't trust politician A to give the cure to every vote when they get power. Even if there's corruption with politician B, there will be distribution regardless. Politician A has an even smaller scope of distribution.

It’s not the same logic at all. You changed the question completely. Politician A will give the cure to everyone who voted for them no matter if they win or not. While B will only give it to everyone if they win. Everyone would logically choose A.
