In Batman v Superman: Dawn of Justice, Superman couldn't see the bomb because Lex Luthor had concealed it inside a lead-lined wheelchair. In Superman's lore, lead is the only substance that blocks his X-ray vision. Because the bomb was encased in this heavy metal, it remained completely invisible to him.

Obviously you didn’t follow Henry cavils Superman very good. The only reason that happened was because he walked in that court trying to be as human as everyone else. So he blocked out his own senses and that’s why he didn’t see it coming.
