On 30 September 2026, Tennessee tried to execute Christa Pike with its one-drug pentobarbital protocol. Her lawyers say she was given both sets of the drug. She survived. On the night of 1 October, they said, she was unconscious on a ventilator. The Department of Correction says it followed every step. The protocol gives no instruction for a prisoner still alive after the second set. In 1995, at 18, Pike and two others tortured and killed Colleen Slemmer, 19, a job-training classmate in Knoxville. Pike was sentenced to death in 1996. In January 2026, her lawyers warned a court that her veins were hard to reach, the drug could leak and the protocol had no plan if an execution failed. The State called her survival "highly unlikely" and said it had "no obligation to attempt revival". A special master and the state Supreme Court called the vein evidence speculative. The department says its drug "has consistently been effective". Of six attempts under this protocol since May 2025, four ended in executions and one, Tony Carruthers's, was stopped before any drug was given. Why Pike's failed has not been established. Her lawyers suspect the drug went into her arms instead of her veins. The Attorney General's office, which approved the protocol, has retained Ed Stanton for an outside review. Commissioner Frank Strada is stepping down. Executions are paused for 2026. Courts have allowed second attempts: Louisiana's in 1947, even if current had passed through Willie Francis, and Ohio's in 2016, because under Ohio law an execution begins when the drug enters the IV line, which it never did for Romell Broom. Tennessee has not said whether it will try again. Sources: TDOC protocol (Chancery Court, Ex. 2); Pike's motion, 2 Oct; Tennessee Supreme Court; DPIC; Nashville Banner; AP; CNN; BBC; State v.
