Pelvic venous disease in hEDS may not always look “classic” on imaging. I see many patients with hEDS who have symptoms suggestive of pelvic venous disease or iliac vein compression—pelvic pressure, low back pain, urinary frequency, pain with intercourse, heavy or aching legs, occasional swelling, and sometimes POTS-like symptoms. Yet CT or MR imaging may show only mild compression or appear unremarkable. This doesn’t necessarily mean anything was missed. Pelvic venous disease in hEDS may behave differently than classic May-Thurner. Veins are dynamic, and most imaging is done lying flat, even though symptoms are often worse upright. Because of that, a supine scan may not fully reflect what happens after hours of standing. So when symptoms strongly fit, I don’t stop at equivocal imaging. I may ask an experienced interventionalist to take a closer look, sometimes with IVUS (intravascular ultrasound), which can sometimes reveal more significant compression than was appreciated on CT or MR. And in appropriately selected patients, treatment can lead to meaningful improvement. This doesn’t replace CT or MR, and not all compression needs treatment—mild narrowing is often incidental.
