Cartilage on the ball and socket has worn away, the femoral head may have collapsed, and the soft tissues around the joint have contracted. Many patients walk into the operating room already several millimeters short on the arthritic side, even when they have never felt it. When we restore that hip with an implant, we're rebuilding the natural anatomy in metal and polyethylene. Length and offset work together to recreate proper soft tissue tension, and that tension is what holds the new joint stable in the socket. Stability is priority one. A perfectly symmetric hip that dislocates is a far worse outcome than a hip that sits a few millimeters long and stays where it belongs. . Robotic and computer assisted planning has changed how precisely we can dial in these decisions. We can verify cup position, leg length, and offset down to the millimeter intraoperatively. A point worth sharing with patients. Perceived length is not the same as measured length. Pelvic tilt, spine stiffness, hip flexion contracture, and years of compensatory gait patterns all influence how a leg feels after surgery. Most apparent discrepancies resolve within six to twelve weeks as the pelvis levels, muscles rebalance, and the brain learns the new normal. When a true discrepancy remains, a simple shoe insert almost always solves it. The goal has never been perfect symmetry on day one. I know that 3/4” to long is bad on the second hip after a fantastic first hip replacement. To bad Dt passed away with cancer before post op appointment form first hip surgery

I had Avascular Necrosis of the femoral head, and it was pretty much gone. By the time I was properly diagnosed and got my hip replacement, my surgeon could only regain so much of my lost length. I am 1/2 shorter and have to wear inserts
