Recent research is identifying mast cell activation as a shared underlying mechanism across all of them. A 2024 study by Van Remoortel et al. in Cell Reports Medicine reviewed mast cell modulation as a therapeutic target for IBS pain, showing mast cells directly drive pain-sensing nerve activation in the gut. Research by Theoharides and colleagues has documented thalamic mast cell involvement in fibromyalgia pain sensitization. A 2025 systematic review found IBS and fibromyalgia share mast cells, pro-inflammatory cytokines, and gut-brain disruption as common mechanisms. The migraine connection comes through CGRP release from mast cells, which is the same target as newer migraine medications. These conditions are now sometimes grouped together as nociplastic disorders. The mast cell connection across these conditions is well-supported in mechanistic research but mast cell stabilizers aren’t yet first-line treatments for IBS, fibromyalgia, or migraine. I've had chronic migraines for years, gut issues for quite literally my entire life but no IBS diagnosis because nobody wants to diagnose me with it (if I do have it I don't want it but anyway), I also get body aches all the time. I just thought this was all just normal shit for me because I have so much wrong with me that life is difficult
