Three former employees of Southern Hills Rehabilitation Center in Tulsa, Oklahoma, have been charged with felony neglect after a 74-year-old resident was found covered in maggots in July 2023. The individuals charged are former licensed practical nurses Audra Owens and Keke Ingram, along with former certified nursing assistant Danisha Brown. Each faces one count of neglect by a caretaker. The charges were filed in Tulsa County District Court on July 10, 2026, and all three are presumed innocent until proven guilty. The resident, referred to as L.T. in court documents, was quadriplegic, nonverbal, and relied entirely on staff for care after suffering a stroke. According to affidavits from the Oklahoma Attorney General’s Medicaid Fraud Control Unit, L.T. had severe pressure wounds and required regular repositioning, hygiene care, and skin assessments. On July 19, 2023, a nurse on the afternoon shift found hundreds of maggots in L.T.'s groin area and bedding, leading to a 911 call. The resident was subsequently taken to Saint Francis Hospital for treatment, with reports that other staff had seen the maggots earlier that day. Court filings reveal significant inconsistencies between the medical records of the resident's care and witness statements about L.T.’s condition. Owens is accused of falsifying documentation regarding catheter and feeding tube care, as well as repositioning the resident, despite witnesses indicating that L.T. was not turned every two hours as required. Ingram, the overnight charge nurse, claimed to perform comprehensive assessments each shift, while Brown initially did not remember caring for L.T. but later acknowledged finding maggots during repositioning and noted a lack of necessary hygiene supplies. The investigation was led by the Oklahoma Attorney General’s Medicaid Fraud Control Unit, which reviewed records and interviewed staff members.
