@shirleneobuobi: I talk about this more in the newsletter that I’m going to release in a few days, and I have so much more to say about the exodus of physicians from outpatient practice. The main thing that I’m mourning is this- I did the best I possibly could under the system we currently have, and it was destroying me. I care deeply about patient care and loved my clinic. What I didn’t love? Everything that came with it that had to do more with profits than people. I wrote about this for the Washington Post in 2022- but being a “good doctor” is mostly only possible under our current healthcare model through total self effacement. As more of us move toward a direct patient care model that sidesteps insurance entirely, or leave clinical practice altogether, the physician shortage is only going to worsen. In the meantime, I am heartbroken for my patients, but so, so excited to be able to get consistent sleep for the first time in 8 years. (I will still be practicing clinically, but only in the inpatient setting - ie seeing patients admitted to the hospital and reading studies. Hopefully this model will work better for me!)

I feel your pain. I am in Direct Primary Care and I felt SO guilty when I made the decision but I came to realize that traditional outpatient primary care was setting me on fire for others’ warmth and the flames would eventually consume me. I can be a really good DPC doc to 500 people and have some energy left to enjoy my life or I could burn out completely and leave medicine. DPC it is! And I love that I can give low cost care to the folks the system leaves behind. Make the decisions you need to make to find something that looks like balance to you. It’s enough. Neither of us is required to bear the burden of a broken system alone. We do what we can do.

I’m an anesthesiologist and I truly can’t IMAGINE how hard it must be to take good care of patients in the outpatient setting. Our healthcare system is ruining the ability for good doctors to be good doctors. I’m glad you did the right thing for YOU ❤️
