When I worked in Peds hem onc and had patients that were difficult to get their pain under control, sometimes on their pca they would write a prn bolus dose order and it was called a nursing dose (bc it was the nurse programming a bolus dose separately from the patient’s button presses) but I hated that they called it that

You can even advocate before you give and say ā€œhey doc I rly think they’re going to need more than thisā€, esp if the dosage or medication seems particularly inadequate! I’ve had a doc order 2mg of zofran when 4mg would be the proper dosage based on the patients weight! Docs make mistakes too

Also – DOCUMENT! PRN ____ admin for ____, dose ineffective. Back yourself up every single time the ordered dose doesn't have the desired effect. Then when you advocate for your patient, and ask the doctor for a dose change, you have the evidence to back it up.

Been a nurse for 15 years, and I’ve never heard of the nursing dose. If I ever felt the patient needed something other than was ordered, I would call the doctor and tell them myself.

I might order a small dose because I don’t know how the patient will react. Don’t give more! I’ll order more if needed but I don’t want to treat a hypotensive, apneic unconscious patient if patient over reacts to the med.

There are nurses I know that would give half instead of the full dose for pain if the pt doesn’t tolerate it well. I still wouldn’t do that. I know some do that if pt requests it. I had a patient that got mad at me for not giving less the prescribed dose because I told them that is something I have to address the doctor since it’s not my call. I can advocate for them but I can’t just do that.

I had a nurse suggest I should give 5 of haldol IM when they were only ordered 1, severely agitated individual. I called the doc and he ordered for us to use 5mg po/im for the future. if i did the ā€œnurses doseā€ then it would look like the agitation was relieved with 1mg, causing a delay in care the next time the agitation happens. so much easier to just get the order! advocate for what you want and the doctors usually agree, they know that we only want what is best for the pt

This was something I’ve seen, but never risked my license for. But this was before Pyxis and wasting was required. If it’s still happening then TWO nurses are not following orders. I won’t waste 6mg of morphine or 8mg of dilaudid without seeing it.

Back in the 90s when I went to nursing school and at the end, you did your little five week haul with a nurse on the floor. Her name is Maria. It was Night Shift and a patient was acting out. She said you guys acting up. I said OK well he can get 1 to 2 mg Ativan. You had to waste a partial dose. She told me that there’s a doctors dose and then there’s a nurse dose. That’s how I learned. šŸ˜‚šŸ˜‚šŸ˜‚

My nurse knew I was in horrible pain after gallbladder surgery incision was made bigger to remove scared gallbladder… nurse called Dr asked for more pain meds.. my hand was scrapping the bedside it hurt so much.. by 3rd day no pain meds just anti inflammatory meds. Post surgery call I THANKED MY NURSES & SURGERY TEAM.. again next post call Thanked them all AGAIN šŸ™šŸ‘
