Go to the hospital clinic with your form and ask for the most experienced phlebotomist. I don’t think they would have a problem.

I have the same problem, i usually ask for the butterfly needle (they use on newborns). It works well.

Same for me. Last blood test they had to use an ultrasound machine to find my vein.

Same. Happens to me all the time but the minute I leave the hospital my veins are all nice and visible and all shades of blue green purple
