@tikdoctony: Want top surgery but worried about losing nipple sensation? That’s one of the most common questions I get. For the right patient, a buttonhole top surgery may help preserve much of the nipple’s original sensation. Instead of completely removing the nipple and grafting it back on, I carefully keep the nipple attached to its blood vessels and nerve supply while reshaping the chest into a more masculine contour. The challenge is that this nerve-and-blood-vessel stalk is incredibly delicate. The longer it is, the greater the risk of stretching or damaging it during repositioning. That’s why buttonhole top surgery tends to work best for patients with smaller chests, where the distance the nipple must travel is shorter. For larger chests, especially D-cup and above, that stalk becomes much longer, making sensation preservation more technically challenging and less predictable. Every patient is different, which is why choosing the right top surgery technique requires balancing sensation, chest shape, safety, and your long-term goals.
